It is difficult for me to do a diagram just now, I have a dodgy tablet.
Basically for forward it is the same as you have got, a wire goes from pos to the switch to the relay to neg, the other side of relay has a thick wire from pos to the relay to the motor and from the motor to earth. You need to cut that earth wire or disconnect it, it goes from earth to the big terminal side of the second relay other big terminal to the motor. The second relay thin wire, coil/switch/energising side is wired in parallel with the first relay.
The reverse circuit is exactly the same but with the motor wires reversed.
Any of you part timers on this site want to do this guy a diagram?

What sort of motor is it?
Some motors run the same way regardless of which way DC is applied (the ones with both wound rotors and stators - as found in starter motors).
